Kentwood Elementary

To enroll your child at Kentwood, please bring the following with you:

1. ORIGINAL Birth Certificate, Baptismal Certificate, Passport, Health/Vital Statistics Affidavit
2. ORIGINAL Immunization Record (Shot Record) . Must include Hep. B series (3) and Mantoux TB test (within past year prior to entry)
3. PROOF OF RESIDENCE Original Electric or Gas Bill, Affidavit, Rental Agreement, or Escrow papers **No Telephone Bill Accepted**
4. PROOF OF LEGAL GUARDIANSHIP
5. PERSONAL IDENTIFICATION
6. REQUIRED FORMS: Orange Health Card (completed by a private physician) and the Oral Health Assessment/Waiver Request Form
